Take Charge of Your Health programs allow you to learn to thrive not just survive with your ongoing health conditions! Find practical ways of dealing with pain and fatigue, discover better nutrition and exercise choices, understand new treatment choices, and learn better ways to talk with your doctors and family about your health. Take Charge of Your Health programs are interactive workshop series meeting for 6 to 8 weeks. Each session meets for 2½ hours weekly. Two trained facilitators, who have personal experience with ongoing health conditions, conduct each workshop.

We invite you to participate in these exciting, interactive programs!

Take Charge of Your Health programs have a successful track record of changing lives. Workshops form in communities throughout the year. If you or your caregiver are interested in attending a workshop, please click the link under the program that meets your need.   • Take Charge of Your Health - Provides information and self-management skills for older adults with a chronic disease and their caregivers
  • Tomando Control de su Salud - A culturally adapted Spanish language program for older adults with a chronic disease and their caregivers
  • Take Charge of Your Diabetes - Provides information and self-management skills for older adults with diabetes and their caregivers
  • Take Charge of Your Diabetes Plus - An eight-week workshop for participants with a diabetes diagnosis. Participants must have a physician referral. The AgeOptions program is accredited by the American Association of Diabetes Educators and is eligible for Medicare reimbursement. ( Currently only available in Cook County)

"Take Charge of Your Health" is the Illinois' branded name for the Stanford Patient Education Research Center Chronic and Diabetes Disease Self-Management programs developed in the 1990s.
