Unit 16 Horse Show

Unit 16 4-H Horse Show: June 11, 2017  at Refuge Ranch in Rochester, IL

In order to participate in the Unit 16 Horse Show, All documents due by May 26th  to the Sangamon County Extension Office or by uploading them to fairentry.com
Required documents include:

  • show registration via fairentry.com
  • 4-H Health and Medical Form
  • 4-H Liabilaty and Risk Release Form
  • Refuge Ranch Liabiltiy Release

There will be drop-in registration Help at the Sangamon County Extension Office
for those needing access to a computer or assistance with registering:
Saturday, May 13th 9 a.m. to noon   OR    Monday, May 22nd from 2 p.m. to 7 p.m.

  • 4-H ASSUMPTION OF RISK AND RELEASE FROM LIABILITY PDF
    Required to participate in Horse Show. All documents due by May 26th to the Sangamon County Extension Office, attn: Kathryn Long

  • 4-H Health and Medical Form Word
    General form, also to be used for Horse Show.

  • 4-H Horse Lease or Ownership Form
    *required for State Fair but not for county/unit show A note for those exhibiting in the IL State Fair Junior Horse Show: They require ownership/lease papers and photographs (the form is available on our webpage as well). Please send Ownership, Lease papers, photos, and Entries for the state show directly to: Pam Harmon at the Illinois State Fair by June 1 for verification of State Fair entries. Your local extension office does not need copies on file any longer. We strongly encourage members, if they own or lease more than one horse, to file paperwork on ALL potential exhibit horses by the deadline. If a horse is injured or dies, it may be replaced ONLY by a horse whose paperwork is already on file by the deadline. A horse may only be owned or leased to, and exhibited by ONE 4-H or FFA member at the State Fair Junior Horse Show. Refer to the Illinois State Fair website and premium book for all other entry and exhibitor information.
