Using this form, enter your monthly income. You should only include income that you know you will receive.
Use your take-home pay amount–your net income after taxes, Social Security, and other expenses have been taken out. This is usually the amount on your paycheck.
Select an income category from the first column, then enter the amount in the second column, and enter the day in the third column. If you are paid several times in the same category, for example, if you are paid more than once a month, then enter each payment as a separate line.
