- 4-H Adult Volunteer Application Form
All adults must complete this application and agree to the screening process prior to being accepted as an adult volunteer.
- Chaperone Training & Certification
The purpose of this website is to provide training for individuals who will be responsible for the health and safety of 4-H youth development participants at overnight events and activities. BY have adult chaperones complete this training, it will provide reassurance to parents for their children's well being. It will also clearly outline the expectations for volunteers who serve in the overnight chaperone role.
- Department of Child Family Services Background Check
All volunteers must successfully pass a background check with the Department of Child & Family Services.
- State Police Conviction Report Request
All volunteers must complete the Conviction Name Report Form through the Illinois State Police.
