4-H Achievement Awards

Different forms needed by members to complete the levels of award recognition.
  • 4-H Key Award Application PDF
    This award recognizes youth who have demonstrated excellence in leadership and a strong focus on community service and mentoring. Open to youth who are 16 to 18-year-olds by September 1, 2016. Winner will receive a plaque and state recognition. Awarded once in a 4-H member’s career.

  • 4-H Veterinary Science Award Application PDF
    This award recognizes youth that have enrolled and demonstrated mastery in the 4-H Veterinary Science project for a minimum of three years. Open to youth who are 15 to 18-years-old as of September 1, 2016 and currently enrolled in the 4-H Vet Science project. Winners will receive a $200 cash prize. Awarded once in a 4-H member’s career.

  • Excel Level Recognition for Clubs PDF
    Any club enrolled with their local Extension Office by August 1 of the calendar year in which they are submitting an application is eligible to apply. The application should reflect the involvement of club members and leaders working together as a team toward the accomplishment of a common goal. The activities and projects must relate to the specific award category and must be completed from September 1 of the previous year to August 31 of the year in which the application is being completed.

  • Excel Level Reocgnition for Club (New replaces the 4-H Club Group Report) PDF
    Any club enrolled with their local Extension office by September 1 of the calendar year in which they are submitting an application is eligible to apply. The application should reflect the involvement of club members and leaders working together as a team toward the accomplishments of a common goal.

  • I Dare You Award Word
    To apply for this award you must be a 4-H member 16 years of age of the current year, or a high school junior or senior. Applicants should demonstrate personal integrity, lead well-rounded lives and possess a willingness to assume responsibility. Applicants should be those recognized as emerging leaders by their peers and adults who work with them. This award is due to the office by September 30th.

  • IL Farm Bureau & Affiliates 4-H Premier LCP Award Application PDF
    This award recognizes youth who have excelled in demonstrating leadership, citizenship, and professionalism in the 4-H work on a local, state, and national level. Open to 4-H members 17 to 19-years-old as of December 1, 2016. Winner will receive a plaque and invitation to Premier 20 Conference. Awarded once in a 4-H member's career. Application is due to the Extension Office no later than January 3, 2017.

  • Illinois 4-H Award Application Word
    Open to 4-H members’ age 15 to 18-years-old as of September 1, 2016. May apply in any of the award areas. Awarded once in a 4-H member’s career. Due to your local county Extension office.

  • Individual Experience Application PDF
    Open to 4-H members to record their experiences during the past 4-H program year (September 1 through August 31). There are four dimensions of experience participation: community service, leadership, and project learning as well as five award levels. Due to your local county Extension office.
