Tree not leafing out small amount of growth of healthy leaves top of tree. Please help!

Oaks can be stressed by environmental conditions, insects and disease. The more stress they are, the more insects find them attractive. Diseases can be those found on foliage, in the trunk or roots. It is likely that your tree has been stressed for several years and last year did not make any new buds for growth in 2015 beyond what you see. Suggest you contact your local Extension Service for a more focused idea of what went wrong and what you may be able to do.

Thank you for your feedback! Very much appreciated! If it is stressed can it be saved or are chances that I will need to remove the tree? It has some leaves but they are sporadic.

Getting someone much more local will confirm the trees condition. Typically twigs and branches that are brittle and snap or have NO green color beneath the bark are dead. You have species of Oak in California that do not grow in Illinois so my comments are from that perspective only.
