Tree Disease?

I have a tree that I'm worried about. Could you take a look at the attached pictures

What kind of tree is this?

What is the 'disease' it seems to have? Is it curable? Seems like it chokes out the branches and kills them over time...

this is a fungal disease called black knot and often attaches plants in the prunus family. The common solution is to prune out the knots several inches below where the knot begins while the tree is dormant. There is no known chemical treatment. Your pictures indicate a widespread condition so pruning may not be an option.
