Both the Illinois Department of Natural Resources and University of Illinois Extension handle calls on a case by case basis. There is not a standard template that is used.
Developing a useful form will depend on how you need to use the information (i.e. do you need to maintain a complaint log, etc.)
When dealing with problems with wildlife it is helpful to know: 1) Is the animal causing property damage or is it considered a public health issue? (Go to question 2) or Does the animal appear to be sick, injured, or orphaned? (Direct the person to a licensed wildlife rehabilitator) 2) If causing property damage-- is the damage being done to landscaping, ponds/water features, vehicles, agricultural production, or buildings on the property? How severe is the damage? or If a public health issue--is the animal perceived as a threat to humans or to pets? Has the animal bitten or scratched people or pets? 3) Does the person know what species of animal is causing the damage? (e.g. has the person seen the animal or identified it using other signs such as tracks or scat) 4) Does the person want advice on how to prevent the problem using techniques such as habitat modification, exclusion, frightening devices, or repellents? (Direct the person to the "Living with Wildlife in Illinois" website) or Does the person want information on having the animal removed? (The 'Find a Professional' page on the "Living with Wildlife in Illinois" website provides contact information for IDNR District Wildlife Biologists and licensed Nuisance Wildlife Control Operators. IDNR DWBs can issue homeowners animal removal permits when the animal is causing property damage. NWCOs will trap and remove animals for a fee. )
